If you’re ready to get started with an evaluation, fill out the form below to “SUBMIT A REQUEST” for an evaluation.
Within 24 hours M-F, be on the lookout for an email from our clinic admin (check spam!) You will receive an appointment time and date. You have 24 hours to pay your invoice or your spot reserved will expire.
OPTION 2
If you’d like to talk to Dr. Kristin prior to booking an evaluation click book a “DISCOVERY CALL”
Here you’ll go over your concerns, ensure R3 is the best fit for you, and get your questions answered.
You will be required to input your info to create an account with Embodia, or the appointment will NOT finalize. The appointment is $25 for Dr. Kristin’s time with you.
